New patients need to referred by their treating physician. Treatment records including the most recent MRI need to be faxed, by that physician’s office, stating the need for treatment. This referral including demographics can be faxed to (541) 681-3088.

The new patient packet will be sent to you via mail. The packet will have what many people refer to as bubble forms. These forms need to be filled out as accurately as possible to assist the physician in treating you. The link to the new patient paperwork contains many of the forms that you will receive in the mail so that you may get started filling them out. Please keep in mind that the papers you will receive in the mail will have more than what is available on this website. The forms available here can replace the exact form found in that packet. Please bring all forms filled out to your appointment.
