Expect the Best Experience

From your first phone call to your follow-up appointments, we want your experience to be a positive one. We’ll work closely with you and your referring provider, as well as your insurance carrier, to ensure a smooth process.

Appointments by referral only

To schedule an appointment, we will need a referral from your physician, nurse practitioner or physician’s assistant, even if your insurance doesn’t require one. Referrals can be faxed to 541-681-3088.

Prior imaging

All patients must have imaging (MRI or CT myelogram) for the area of concern within the last six months. If you had imaging at a location other than PeaceHealth, Oregon Imaging Centers or Willamette Valley Imaging, please bring a CD of your images to your appointment.

When to arrive and what to bring

For all new patients, we request that you arrive at our office 30 minutes prior to your scheduled appointment time. All established patients, please arrive 15 minutes early.

Items to bring to your appointment:
• Photo ID
• Insurance card(s)
• Insurance co-pay
• Current list of all medications
• CD of images, if needed (see above)

Patient packet

Please fill out and mail us your completed patient packet no less than five days prior to your appointment date. If you are unable to mail within this time frame, please bring a completed, printed copy to your appointment.

Appointment cancellations and rescheduling

If you need to cancel an appointment, please contact us at least 24 hours prior to your scheduled time. Please note that if your appointment is canceled within 24 hours, you will be marked as a no-show. Office hours are Monday through Friday, 8 am to 5 pm.

Insurance, billing and payments

Currently, we are not accepting Workers Compensation or second opinions. All billing inquires and payments should be made to PeaceHealth by calling 541-984-4301.

Prescriptions and refills

Please contact your pharmacy for refill requests and allow 48 business hours for your prescription to be processed. Narcotics require a printed prescription that must be picked up in person at our office; please call us and allow 48 hours for staff to prepare this prescription for you.

Who to contact after-hours

If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
